From the death of David "Rocky" Bennett on a secure hospital ward, to the daily suicides and self-injuries taking place in Britain's prisons, the mentally ill are being failed, reports Sophie Goodchild

PSYCHIATRIC PATIENT David “Rocky” Bennett spent the last 28 minutes of his life pinned face-down on the floor of a hospital ward by a team of at least four nurses. Staff at the Norvic clinic in Norwich rushed to restrain the 38-year-old Rastafarian after he attacked another patient who had racially abused him then he punched a female nurse. They did not release him until he “went quiet” but by then it was too late to resuscitate the schizophrenia sufferer.
