A foundation hospital chairman has been sacked by the independent regulator for failing to address his NHS trust's financial difficulties.

John Ryan, chairman of Bradford Teaching hospitals, was fired for not producing a credible recovery strategy after accountants found an £11.3 million hole in the accounts.
