The National Audit Office has begun an inquiry into the Inland Revenue's failure to stop millions of taxpayers filing late or inaccurate self-assessment tax returns.

The investigation follows criticism of the Revenue after the NAO revealed earlier this year that it had failed to collect £14 billion in unpaid taxes.
