Witness confusion blamed for ballot losses
The loss of up to 60,000 postal votes in northern England in last week's elections took place because of apparent confusion over the need to get them witnessed.
The Electoral Commission is set for an investigation into the problems.
More than 85,000 votes were invalid in the four regions using postal votes says the Guardian.
