The North West London Integrated Care Pilot wins at this year's HSJ Awards

The North West London Integrated Care Pilot (ICP) has been recognised for its exceptional work in bringing together the care received by people aged over 75 and those with diabetes at the 30th annual Health Service Journal (HSJ) awards.

Last night the pilot fought off a strong shortlist to pick up the top award in the managing long term conditions category. The project was commended for its innovative approach in ensuring patients get timely, appropriate care that is focused around their long term needs.

Dr Andrew Steeden, local GP and Clinical Director for the ICP said: "It's great news that our exciting new project has been rewarded in this category. The pilot is working hard to improve the quality of care delivered to these groups, by joining health and social care workers into local multi-disciplinary teams. These teams are then responsible for ensuring that each patient has a care plan that sets out steps for maximising the standard of the care that they receive.

Dr Steeden added: "Each group of clinicians should be congratulated on all their hard work so far, their leap of faith and for working in a new way, with new partners, putting the patient where they should always be, right at the centre of care planning."

Launched this year the North West London ICP is clinically-led by GPs, hospital doctors and community care professionals. It involves a partnership between local NHS organisations, local authorities and relevant charities. Boundaries are lifted between providers so they can work together as a team allowing patients to receive the right kind of treatment, in the right place at the right time.

James A. Reilly, Chief Executive of Central London Community Healthcare NHS Trust said: "We are delighted the work of the North West London Integrated Care Pilot has been so highly recognised. The coordinated care planning in this ground-breaking pilot is supporting more people in their homes and communities, so avoiding unnecessary hospital stays.

"This scheme clearly demonstrates that the seamless, integration of primary, community and other health services and adult social care is vital to improve the quality of care for patients and deliver better value for the NHS."
