Central London Community Healthcare NHS Trust (CLCH)

CLCH is London's largest community healthcare NHS Trust, and works within the boroughs of Barnet, Hammersmith and Fulham, Kensington and Chelsea, and Westminster.

We provide community and in-patient services to more than 950,000 people. A large proportion of CLCH's services are delivered in patients' homes, but we also operate in more than 160 locations including schools, health clinics, GP surgeries and residential care homes.

Our main adult services are district nursing and community rehabilitation (which supports patients discharged from hospital). CLCH works with patients to manage long-term conditions including diabetes, lung issues and disease, heart disease, chronic wounds, pressure ulcers and continence. CLCH's main children's service is health visiting. We also run a number of nurse-led walk-in and urgent care centres, and provide offender healthcare at HMP Wormwood Scrubs.
